Independent Study Course Form
Fill out this contract for an independent study project describing the learning goals, general methods, anticipated project or research results, product student will provide, grading expectations and arrangement for student-faculty interactions.
Master's Program Forms
M-1: Plan of Study
Form your Advisory Committee and then have a plan of study meeting with your Advisory Committee prior to completing 9 credit hours or two semesters of classes (whichever comes last). Once your Advisory Committee approves your plan of study, submit this form to the Director of Graduate Studies.
M-2: No Form
M-3: Thesis or Scholarly Project Proposal
After you have developed a complete thesis (Plan A) or scholarly project (Plan B) proposal in consultation with your Major Advisor, hold a proposal meeting with your Advisory Committee. Once the Advisory Committee approves your proposal, submit this form to the Director of Graduate Studies.
Doctoral Program Forms
D-1: Plan of Study
Have a plan of study meeting with your Advisory Committee prior to completing 9 credit hours or two semesters of classes (whichever comes last). Once your Advisory Committee approves your plan of study, submit this form to the Director of Graduate Studies.
D-2: Dissertation Plan
Within 60 days after attaining candidate status, develop a dissertation plan in consultation with your Major Advisor and submit this form to the Director of Graduate Studies.
D-3: Dissertation Proposal
After you have developed a complete dissertation proposal in consultation with your Major Advisor, hold a proposal meeting with your Dissertation Advisory Committee. Once the Dissertation Advisory Committee approves your proposal, submit this form to the Director of Graduate Studies.
